The Health Information Continuum is a system for collecting and processing HI data for individual patients across multiple health care provider interactions. The system expands the amount and type of data collected by current HI systems to create a more complete representation of a patient’s health. Furthermore, the broad scope of information collected by the Continuum can provide robust summaries of a patient’s health and well-being as well as predictive alerts pertaining to potential health issues.

 

Background:

Electronic health records (EHRs) accounted for the largest share of the market (61.5%) in 2013 and the market is expected to grow to approximately $19.7 billion in 2019. Virtually all eligible hospitals and over 85% of all eligible physician practices are expected to have implemented EHRs by the end of 2019. Few recording services focus on improving the detail of individual patient records or the scope of data used to create health assessments, both being areas where The Health Information Continuum excels.
